Effect of bromhexine in hospitalized patients with COVID-19 - Journal of Investigative Medicine

Image
Significance of this study What is already known about this subject? The COVID-19 pandemic remains one of the major public health issues, despite preventive measures being implemented worldwide. Bromhexine is a potent inhibitor of transmembrane serine protease 2 and has an antiviral effect. One study has shown the clinical benefit of this inexpensive medicine in patients with COVID-19 pneumonia. What are the new findings? Bromhexine is not an effective treatment in hospitalized patients with COVID-19. Presence of renal disease is the strongest predictor of mortality in hospitalized patients with COVID-19 in our multivariate analysis. Bromhexine as a transmembrane serine protease 2 inhibitor could not reduce duration of hospitalization. Bromhexine as a transmembrane serine protease 2 inhibitor could not reduce the need for mechanical ventilation compared to the control. Manageable Safety/Efficacy With Erlotinib/Binimetinib in EGFR+ or KRAS+ NSCLC - Targeted Oncology

Image
Andreas Saltos, MD, discussed treatment options for EGFR and KRAS-mutated NSCLC, and the investigational use of binimetinib to treat these tumors. KRAS or EGFR mutations can be found in up to 40-50% of patients with non-small cell lung cancer (NSCLC). Because of this, a phase 1/1B trial (NCT01859026) was created to examine the efficacy and safety of the combination of erlotinib (Tarceva) and binimetinib (Mektovi) in this patient population. Within the phase 1 dose escalation portion of the trial, patients were administered binimetinib orally on cycle 1, day 1, along with erlotinib on cycle 1, day 2. Binimetinib was dosed once daily or twice while erlotinib was dosed daily on a 28-day cycle. Then, the dose-expansion phase 1b portion of the trial will have 2 experimental groups who each will receive binimetinib 2 times a day, and erlotinib once a day on 28 day cycles. Across phase 1 and 1B of the trial, a total of 43 patients aged 18 years or older with a histologic or cytologic diagnos

However, some troublesome side effects, including bleeding and heparin-induced thrombocytopenia, often occur with the use of LMWH. 2,3 Warfarin, another commonly used anticoagulant, has many drawbacks, including the need for routine coagulation monitoring, frequent dose adjustments to maintain safe and effective levels, a narrow therapeutic window, a slow onset of action, and multiple food and drug interactions. 4,5 Hirudin, a direct thrombin inhibitor, is more effective than heparin in preventing and treating arterial and venous thrombi, 6,7 but the use of hirudin is often accompani
